Tropical Travel Trouble 009 Humongous HIV Extravaganza
LITFL • Life in the Fast Lane Medical Blog LITFL • Life in the Fast Lane Medical Blog - Emergency medicine and critical care medical education blog aka Tropical Travel Trouble 009 The diagnosis of HIV is no longer fatal and the term AIDS is becoming less frequent. In many countries, people with HIV are living longer than those with diabetes. This post will hopefully teach the basics of a complex disease and demystify some of the potential diseases you need to consider in those who are severely immunosuppressed. While trying to be comprehensive this post can not be exhaustive (as you can imagine any patient with a low ...

Dyspareunia in HIV-positive and HIV-negative middle-aged women: a cross-sectional study
Conclusions Dyspareunia was common in this group of HIV-positive women and was associated principally with vaginal dryness and urinary incontinence. The importance of treating dyspareunia within the context of sexual health in this group of women should be emphasised and appropriate management of this issue may reduce the likelihood of lesions on the vaginal wall, which may act as a portal of entry for other infections.

Bone Loss Among Women Living With HIV
AbstractClinical data accumulated over the past two decades attests to a significant decline in bone mineral density (BMD) in patients infected by HIV, which does not remit but may actually intensify with anti-retroviral therapy (ART). Long generally perceived as an aberration without clinical consequences in relatively young HIV-infected cohorts, recent studies have documented marked increases in fracture incidence in HIV-infected men and women over a wide age continuum. Fractures are associated with chronic pain, crippling morbidity, and increased mortality, undermining the gains in quality of life achieved though ART. A...

Chronic Pain and Opioid Use in Older People With HIV
Top Antivir Med. 2022 Dec-Jan;29(5):419-422.ABSTRACTChronic pain is common among older people with HIV. Etiologies of chronic pain are multifactorial in this population. A careful and thorough initial assessment of pain is important. Associated conditions that can contribute to pain should be explored and managed as indicated. Special consideration is warranted for some of the unique aspects of pain in people with HIV. Chronic pain management is multimodal; a variety of pharmacologic and nonpharmacologic strategies are effective. Among medications, opioids can be used but carry a risk of significant harms. The use and moni...

Understanding the Etiology and Management of HIV-Associated Peripheral Neuropathy
Abstract HIV may cause several forms of peripheral neuropathy, the most common of which is distal symmetric polyneuropathy (DSP) characterized by pain and sensory deficits in a stocking-glove distribution. The pathophysiology of DSP remains largely unknown but is thought to be related both to the neurotoxicity of HIV—through indirect immunomodulatory mechanisms—and to the neurotoxic effects of anti-retroviral therapies, most notably the dideoxynucleoside reverse transcription inhibitors or so-called d-drugs. Determining whether symptoms arise from the virus or the treatment poses a challenge to the clinician ...

Patient acceptance of HIV testing services in rural emergency departments in South Africa
Conclusions: Despite national guidelines, a high proportion of individuals remain undiagnosed, of which a majority are young men. Our study demonstrated high patient acceptance of ED-based HCT. There is a need for investment and innovation regarding effective pain management and confidential service delivery to address patient barriers. Findings support a routine, non-targeted HCT strategy in EDs.

Neuromyelitis optica in hiv: a case report and literature review
We present a case and review the literature. A 49 year old lady of African descent with known HIV diagnosed in 2004 – compliant with her antiretroviral medication, Atripla – was admitted with a 3 week history of thoracic back pain, spreading paraesthesia and weakness involving both legs, and constipation. Examination revealed a spastic paraparesis, a sensory level at T3/4 on the right and reduced vibration sense to the iliac crests bilaterally. MRI spine revealed increased signal within the cord from T3–7 in keeping with a longitudinally extensive transverse myelitis. CSF was acellular with normal protein...

P108 Early Integration of the Palliative Approach in HIV Management: Refining a Curriculum for Non-palliative Specialists
Human immunodeficiency virus (HIV), a chronic illness for those accessing treatment, requires a paradigm shift moving from viral measurements alone to a perspective that encompasses quality of life as defined by the index person and his support system. HIV positive young men who have sex with men are at high risk for poor outcomes and for spreading disease, in part, because they are difficult to engage and retain in care. Outpatient staff themselves may be a contributing factor. We employed the palliative approach early (ePA) to assist non-palliative outpatient staff in improving outcomes for a difficult to engage and retain HIV population.

Argon plasma coagulation treatment of anal high ‐grade squamous intraepithelial lesions in men who have sex with men living with HIV: results of a 2‐year prospective pilot study
ConclusionsAPC can be used to treat anal HSILs in HIV‐seropositive MSM, and requires repeated treatment because of a high recurrence rate. As successful treatment of human papillomavirus (HPV) infection or eradication of the anal transitional zone remains impossible, HSIL treatment is challenging and requires long‐term follow‐up.
